Hospice care is for the patient who has a terminal illness, which is anticipated to result in a life expectancy of six months or less, as determined by the physician. The decision to choose hospice is made in collaboration with the family, physician, and hospice team.
Patients may remain in the familiar and comfortable surroundings of home, whether it’s a private residence, nursing home or an assisted living facility. Once a decision has been made, the patient signs a statement indicating that the goal of treatment will be comfort care, not curative in nature. The patient is able to continue under the care of his/her own attending physician.
Hanover Hospice’s team is specially trained in end-of-life care. To meet the needs of the patient and family, the team is available 24-hours a day, 7-days a week. A licensed nurse will answer your after hours calls. Hanover Hospice does not use an answering service.
